You can do it without DSL or other high-speed Internet access, but personally I don't recommend it. If you don't go with a TASP that has a fully-integrated Internet portal, then yes, you'll need a FTP host and software, and you'll probably need to purchase encryption software. ( I think there are some free encryption programs but I got so entangled in the encryption keys, and having the doc's office install the software and get encryption keys that I finally gave up on that!)
