Per page rate is hard to specify

depends too much on margins, the "density" of the lines in terms of how many characters, etc.

I have an account with 3 doctors and 2 nurses, all using the same format, same margins, etc.

I charge cpl for this, not by the page. But, when I figure out how much I get per page for this work, it can vary from $9/page up to $14/page, depending on the dictator and how "dense" the pages are. The $14 guy dictates very long lines. The $9 guy dictates short notes, short lines.

I have another account that I charge by cpl but those pages turn out to be $6 to $7/page because that doctor's format is different, more white space on the page, etc.

If you're only doing transcription for one doctor, then maybe you could figure out an "average" page rate to charge, but I wouldn't suggest doing that for an account that has more than one dictator (unless, of course, you can have a different page rate for each dictator).
