This bright-eyed 58-year-old female presents today seeking employment as a Medical Transcriptionist.
HISTORY OF PRESENT ILLNESS
Charlotte has been a resident of Goodyear, Arizona, for six years and works as a Medical Transcriptionist, presently volunteering her services to John C. Lincoln Hospital, and having recently completed an intensive eight-week internship at Walter O. Boswell Memorial Hospital after a twelve-month medical transcription course at Estrella Community College.. Charlotte wants to do even more transcription; preferably at home, and is able to do this while keeping her present commitment of service to the hospital.
REVIEW OF SYSTEMS
Charlotte, as she prefers to be called, has had two years of community college in business and then worked for 13 years for an aerospace company preparing proposals and business plans. In 2001, she retired and then chose to enter the medical field doing what she loved best: Medical Transcription. She is the first graduate from Estrella Community College, Southwest Skill Center to receive the Certificate in Medical Transcription.
PHYSICAL EXAMINATION
GENERAL: A well-developed, well-nourished female in obvious distress seeking employment.
HEENT: Head - normocephalic, atraumatic, obviously full of medical terminology. Eyes – alert, keen sense of hearing, and can easily distinguish between opathy and apathy, even if physician has a foreign accent.
CARDIAC: Regular rate and rhythm. No hesitancy, no gallop, no murmur. Loves her work.
EXTREMITIES: Hands - quick, accurate, types 65 w.p.m. Feet - have good control of dictation equipment foot pedal. Back and pelvis – can withstand long hours sitting performing transcription.
