1. Doctor dictates the date of the exam is "March" 30, 2005.......I'm fairly certain she MEANT APRIL.

2. DRUGS:
....says clearly "citaprolam"....does she mean ? citalopram?

.....says clearly "namebutone"...does she mean ?nabumatone?

3. Last week my friend had RX filled at drug store. She looked
at doc name on bottle and it was NOT HER DOCTOR. She
called pharmacist. Pharmacist, "Oh, we couldn't read the
doctor's writing, so we guessed."

Do you know how sick of medicine I'm getting. Sloppy is not even covering it any more. Try CRIMINAL.
