Need advice, am I a bad boss

Okay, I hear all the gripes about the pay,quality and so on. I am a very small business and have had to use subcontracts occasionally. The one I am using, and is also a friend, who went through some MT classes and over embelished her qualifications but anyway I though that I could take the basics and build. She had beena top notch legal secretary and I thought had good basic sense. She has been with me for almost 10 months, and yet when a doctor uses a drug, or an exam that she at first cannot understand but then he uses it again later on and she gets, she does not go back and realize that gee these this sure ounds like what is above. They are identical. She cannot get Augmentin in the subjective where it states, "patient was given ???? by phone on 4-15. Then in the plan it agaiin states, Patient was given Augmentin by phone on 4-15 and she will complete the 10 day course -- should she not pick up on this? Or if she had a patient two days ago and we corrected the idential items that she is now typing on, wouldn't you think that she this would trigger her memory? I pay her very well, what some of you are making at a National with years of experience as they really need the money but I am getting to my witts end. ALso I know kids and family and church are important but it is hard to get her to realize that this is a job and it needs to be done by certain deadlines. Also when she waits until 9 to 10 to type it, she has a lot of mistakes as she is so tired. So am Ijust being a picky boss? I go over and over with her, have bought her many machines, we are still on tapes but the time it takes me to go over her work, I could type it. Hopefully we are going digital in a month so I can then at least have the dictation to listen to when there is a question. But I feel like she does not want to learn or listen. She also wants to work 4 hours a day, three days a week and earn $1000 to $1200 a month and ony work Tu, Wed and Th and wants to pick up another account so she can work those three days and stick me with the other two.
