for macrosomia, absolutely no way I could have the baby vaginally, was in induced labor with first baby 27 hours before having a C-section. C-section was scheduled, I quit work a couple of days beforehand to have a couple of days to relax and 12 hours before surgery insurance company denied it, said I needed a trial of labor first. Had to cancel surgery but my doctor wrote the insurance company a letter saying that he would support me in a lawsuit if my uterus should rupture from labor and that 7 lbs was the biggest baby I could deliver vaginally, this one was at least 8 and +3 station. They finally agreed to surgery about a week later and baby was 9 lb 15 ounces.

The doctor had called the insurance company before writing, but said the guy had such a foul mouth he hung up on him and wrote the letter and faxed it.

For some reason as yet unknown to me, insurance companies are supposed to deny a certain amount of claims, even if they are valid and a lot they end up paying when challenged. As much as you pay for insurance it is a crock.
