It was usual the MTs in our hospital knew of the medical screw ups way before admin did, if the admin EVER did.

The docs would ride the fine line of "reporting" anything by dictating the mess up using confabulation and consider that as notifying admin, leaving it up to authorities to "see it".

Of course, we would have typed it and recognized the faux pas immediately (if we weren't sleeping at the keyboard), but since we are scolded for ever stepping away from the computer and are treated as ignorant uneducated re-entering housewives making extra money for the family ---------(groan)-------- our unique knowledge and what could have been an OBVIOUS cat-bird seat for admin. was spit on, well in my estimation that's how we were treated if we ever came forward with much. I can recount case after case of such situations.
We knew when wrong lungs were entered, wrong patients were operated on with the wrong procedures, wrong extremities operated on, patients sliding off the x-ray table during a procedure (ugh!), oversedation of patient with wrong dose, giving patient medication that was clearly marked as the patient being allergic to said drug, contrast dye being administered to patient when it was marked DO NOT GIVE CONTRAST DYE.

Case after case, buddy! It's there. Just look for it.
