I have had such a terrible time with heel pain after transcribing with shoes, that I had to have an injection in my right heel a while back. It got a little better for a while, but I can still feel the pain sometimes. I use a foot rest under my desk because I tended to get some leg swelling from sitting so long, and I think that the pressure on the heels is greater with that actually because of where the pressure is when I press and release the pedal, if that makes any sense. So what I do now is I use one of those long gel wrist rests along the bottom of the foot rest with my foot pedal above that, so my heel rests on that and doesn't cause as much pain.
