Figure out what you're making an hour for this guy to see if all the little extras are worth it for you. You can type the letters or reports for him, then do a merge or cut and paste on the doctors' names and addresses, and it's just a quick reprint. Then charge him $1 for a reprint or something to that effect. You didn't actually have to retype the entire report, but time, paper and ink cost money, too.
