TAT question(sm)

There seems to be more of an emphasis lately on meeting TATs. Has anyone else noticed an increase in urgency? Who decides what the TATs for each work type will be? I remember way back when I worked in the hospital somebody (don't remember who) decided that the OP notes were to be dictated and on the chart within four hours of the operation. However, to this day, many years later, there are still surgeons who dictate way past that time. And how are the TATs for a service determined? Are they using their own criteria or going by some other?
