Electronic medical records header

I have acquired an account that uses electronic medical records. Because of this, they need a special header at the top of each new patient letter/report. Does anyone already have a macro or something that will automatically insert this in for each new patient? Or is there is better/quicker way to get this accomplished?

Any advice would be appreciated.
